Broke my iPhone 4 will Verizon insurance charge me? ?

I got an iPhone 4 a few months ago and I dropped it a month ago and the screen is cracked. I can still read it but it's broken and lumpy feeling. I purchased insurance will Verizon replace my phone at Tge store or will they send it to me. Also will I have to pay a $50 fee to get another?
